PGY1 Pharmacy San Rafael Area, California
The Kaiser Permanente San Rafael Area PGY1 residency program aims to build upon the Doctor of Pharmacy (PharmD) education and outcomes to develop pharmacist practitioners with knowledge, skills, and abilities as defined in the educational competency areas, goals, and objectives. Residents who successfully complete PGY1 residency programs will be skilled in diverse patient care, practice management, leadership, and education, and be prepared to provide patient care, seek board certification in pharmacotherapy (i.e., BCPS), and pursue advanced education and training opportunities including postgraduate year two (PGY2) residencies.
Pharmacy Services
The Kaiser Permanente San Rafael Pharmacy Services department is comprised of talented pharmacists and supportive personnel in the inpatient, outpatient, and ambulatory care practice settings. Our hospital pharmacy includes a new USP 797/800 compliant IV clean room. In addition, we have five outpatient pharmacies and a new, state-of-the-art outpatient oncology infusion center pharmacy. By using enterprise-wide outpatient pharmacy computers and electronic medical record systems, we are able to provide world-class, integrated patient care for our members.
Each pharmacy service line maintains a strong, collaborative relationship with our group-practice physicians. The inpatient pharmacy department provides clinical services in acute and critical care medicine as well as an award-winning antimicrobial stewardship program. Ambulatory pharmacists provide clinical services that include cardiovascular risk reduction, medication therapy management, drug use management, chronic pain and harm reduction, fracture prevention, infectious diseases (Latent TB and PrEP), oncology, geriatric pharmacy, asthma, and anemia care, psychiatry, heart failure, and transitions of care pharmacy services. Our outpatient pharmacy services provide over 1.2 million prescriptions for our members each year. In addition, our pharmacists take an active role in a variety of interdisciplinary leadership committees at the medical center, including the Pharmacy & Therapeutics Committee, Medication Safety Committee, and the Medical Center Drug Utilization Group.
At Kaiser Permanente San Rafael, pharmacy practice residents play an important role on our integrated healthcare team by providing pharmaceutical care for our members across the continuum of care. Our program’s success stems from our constant drive for performance excellence.
